This page provides forms that current residents may submit to the Housing Office. For information about policies related to living in residence, please refer to the Residence Handbook. For information about the Meal Plan and Visiting Scholar Meal Program, check out the Meal Info page.
Maintenance & Supply Request Form (MSRF)
The resident is responsible for submitting their own maintenance and supply requests to the UW Housing Office. Print this form out and fill in with a description of your maintenance request. Copies of this form are also available from the Housing Office.
Maintenance. Examples of regular maintenance requests include leaking pipes, broken bed frame, malfunctioning fridge, etc. For emergencies such as fire or severe water leakage please directly contact Chris Harwood in the Housing Office. Emergency issues (affecting building and resident safety) will be given priority over all other issues. (Note that if resident abuse/misuse of appliances or furnishings is suspected, the resident may be liable for cleaning and/or replacement costs.)
Supplies. The M/S Request Form may also be used to request replacements for consumable items such as toilet paper and light bulbs.
Party and Event Request Form
If you want to have a party in residence (any gathering of more than four [4] people), you must submit a party request form at least seven [7] days prior to the party. You may submit the online form below to have your party authorized by the Housing Office.
Contract Early Termination
Residents have two options for terminating their contract early, depending on the reason why and when it is done:
Early end to course of study. Those students whose course of study ends before their default contract end date (either April 22 or August 25) may apply no later than September 30, 2011 using the “Request for Early Termination – School Ending” form. This form must be submitted in person at the Housing Office.
This form will be made available Fall 2012
Request for early termination. For all other reasons other than early end of studies (such as found another place, moving to another school, etc.), residents can apply using the “Application for Contract Early Termination” form by the Term deadlines specified on the form. Note that this option requires forfeiting the damage deposit, and payment of a $350 Early Termination Fee and fees for the actual length of stay. Meal Plan subscribers may also incur a meal plan cancellation fee, as per the terms of the Meal Plan contract. Applications must be submitted in person at the Housing Office.
Payment Plan Application Form
This form is filled out as part of the application for a monthly payment plan. Note that the form must be accompanied by payment of the Payment Plan Setup Fee (see details on the form).
